Cerebrospinal fluid oligoclonal immunoglobulin gamma bands and long-term disability progression in multiple sclerosis: a retrospective cohort study

چکیده
Abstract Multiple sclerosis (MS) patients with immunoglobulin gamma (IgG) oligoclonal bands (OCB) in the cerebrospinal fluid (CSF) have different genetic backgrounds and brain MRI features compared to those without. In this study, we aimed determine whether CSF-OCB status is associated long-term disability outcomes. We used Swedish MS register data on clinically definite known OCB status. Date of birth, age at onset, time sustained Expanded Disability Status Scale (EDSS) milestones 3, 4, 6; conversion secondary progressive (SP) MS, sex, immunomodulatory treatment (IMTs) duration were collected. Multivariate Cox regression models investigate association between risk reaching each milestone. The OCB-positive group reached an earlier younger age. OCB-positivity significantly increased EDSS 3.0 (HR = 1.29, 95% CI 1.12 1.48, P < 0.001) 4.0 1.38, 1.17 1.63, 0.001). had a 20% higher SPMS. presence Our findings suggest disease modifying effect early inflammatory stages .

Cerebrospinal fluid oligoclonal bands and progression of disability of multiple sclerosis.
Antibody-mediated inflammation is believed to contribute to tissue injury in multiple sclerosis (MS). The majority of patients with MS have oligoclonal bands (OCB), corresponding to antibodies against a variety of antigens, in their cerebrospinal fluid (CSF). The relation of CSF OCB and disease progression in MS is uncertain. To investigate whether there is a relation between CSF OCB and a more...
متن کاملMyelin basic protein, oligoclonal bands, and IgG in cerebrospinal fluid as indicators of multiple sclerosis.
There currently are three clinical laboratory procedures for use with cerebrospinal fluid that assist in the diagnosis of multiple sclerosis: measurement of myelin basic protein and IgG, and demonstration of an oligoclonal band. We compared characteristics of these procedures, using CSF samples from 166 patients identified as having (54 patients) or not having (112 patients) multiple sclerosis....
متن کاملOligoclonal Bands in Cerebrospinal Fluid of Black Patients with Multiple Sclerosis
Genetic susceptibility is a well-recognized factor in the onset of multiple sclerosis (MS). The objective of this study was to determine the frequency of oligoclonal bands (OCB) restricted to the cerebrospinal fluid, in an ethnically mixed group of MS patients in the city of São Paulo, Brazil. Techniques used to detect OCB consisted of isoelectric focusing followed by immunoblotting. OCB were f...
متن کاملOligoclonal bands in cerebrospinal fluids: significance of corresponding bands in serum for diagnosis of multiple sclerosis.
Controversy exists regarding the definition of oligoclonal bands in cerebrospinal fluid (CSF) and whether CSF bands with corresponding bands in the serum should be disregarded in the interpretation. Much of this controversy results from not distinguishing between the sensitivities of different techniques used for these studies. Because the combination isoelectric focusing/silver staining is mor...
